2010 Option Period Material
Notice to Medicare Eligible Members with HealthChoice Life Insurance
The pre-printed Option Period Enrollment/Change Form for some members may indicate an incorrect amount of life insurance. Corrected forms are being mailed to members who may be affected by this error. If you are making changes during Option Period, please use the Key Points (form instructions) that were included with the original form. If you are not making any changes, please retain this form as proof of your benefits that will be in effect January 1, 2010.
This error does not affect members who are eligible for a Medicare Advantage Prescription Drug Plan.
